Dr. Sandeep Nayak, a leading oncosurgeon in Bangalore, shares a case study highlighting the efficacy of Robotic-Assisted Breast-Axillo Insufflation Thyroidectomy (RABIT) and Robotic-Assisted Thoracic Surgery (RATS) in treating a large multinodal goitre that extended into a patient’s chest, medically called retrosternal or substernal goitre.

Dr. Sandeep Nayak, a surgical oncologist par excellence, has developed a revolutionary technique called Robotic-Assisted Breast-Axillo Insufflation Thyroidectomy (RABIT) to treat thyroid issues needing surgical intervention. RABIT is a minimally invasive procedure that is less painful than open thyroidectomy, which leaves large scars and is more painful.

Large retrosternal goitres usually require the chest to be opened with a sternotomy wound which is painful and has complications associated with it. Performing this surgery using robot surgery makes it less painful and allows the patient to recover faster from this complex surgery.

Moreover, the patient can recover faster with small incisions and less surgical trauma. In addition, there is minimal scarring after the procedure compared with the visible scar across the neck in the case of open thyroidectomy.

Referring to his case story, a 65-year-old woman approached him with a complex retrosternal goitre for treatment. The goitre of size 15 cm, with 10 cm, extended into her chest. She had several blood vessel anomalies that further complicated her condition.

Upon extensive examination and discussion with the multidisciplinary team, Dr. Sandeep Nayak settled on integrating the RABIT and RATS procedures to remove the goitre. The RABIT procedure was performed to remove the neck portion of the goitre, while its thoracic section was removed using the RATS procedure. Dr. Sandeep Nayak performed both these procedures successfully using the da Vinci Xi Robotic Surgical System. The surgical team successfully maneuvered the robotic arms to gain access to the goitre and remove it without harming the surrounding tissues, organs, and areas. She was extensively monitored for no postoperative complications. The patient recovered well and showed great improvement in her respiratory function.
